A Sheriff is the highest elected official in a county, the under sheriff is directly under the sheriff and helps with running the sheriff's department. kind of like President and vice president. If president can not take care of dutys, the vice president will.

The Chief Deputy in some states is called the Undersheriff. Same thing, like the second in command.
Some bigger departments have a full chain of command down to the street level.

He is like the head deputy(ies). Many years ago a Sheriff only had to manage 1 to 3 deputies. Many populous counties in the metropolitan area of a larger city may have 100 or more deputies to manage, so the Sheriff appoints 1 or more Under Sheriff(s).
