(Total cost: $7 per person for the subway, $12 for the LIRR.) There are also buses, like the already mentioned Super Shuttle or the New York Airport Service buses that go to Grand Central, Port Authority, Penn Station and a number of midtown hotels.
Fastest is probably to take a cab. Make sure you get a cab from the taxi stand and it's a yellow cab. There's a $45 flat fee from JFK to Manhattan or you can ask the cabbie to run the meter, but it would probably work out to about the same.
