Columbia, it was the first fully functional Space Shuttle, you could call it the first one, although there was one prior to it called Enterprise, but it was used only to test short flights, without leaving the atmosphere.
Columbia desintegrated attepmting re-entry to earth's atmosphere, and the pieces were sparced all over the midwestern states.
