I presume you mean severe atrophy of the cerebral cortex. Depending upon the age, it can present with mental retardation or dementia. There may also be loss of function of the particular area involved,

Severe cortical atrophy means that a large portion of the cells in the cortex (I presume, you are referring to the cerebral cortex in your question) have died. This typically happens as a result of a blood-clog in the blood vessels in the brain, a brain haemorrhage or a severe blow to the head.
It is not possible to say how this will present itself, since that depends on what parts of the brain is affected. However, with serious atrophy, you will usually see difficulties in concentration, attention, mental processing speed, spatial orientation and memory.
