The differential diagnosis for this is too wide without more information.

I have seen this sort of thing in:

adrenal insufficiency
inborn errors of metabolism
insulin overdose
severe sepsis
...

more details please: age of patient, past history, current location? (ICU I'd hope if you are truly describing profound hypoglycaemia), treatments instituted, concurrent illnesses, how did this present?

Maybe the diagnosis should go from hypoglycemia to diabetes and be treated as diabetes is treated.
Consider other meds that can cause hypoglycemia, for instance an antibiotic I am on can cause hypoglycemia.
Consider other conditions that can affect the blood such as anemia.And even a painful condition can, as in PAIN, cause changes to the blood.
Stress can be a factor. Smoking can be a factor.
This one is not a cut and dried type of illness and treating it is difficult until the reason for it has been singled out. But that is only by ruling out everything else, one at a time, which is lengthy and prolongs the final diagnosis.Hang in there, keep hope, relief is available.

persistent profound hypoglycemia ... especialy fasting hypoglycemia raises a possibility of an Insulinoma. whic can be detected by blood tests along with some imaging.
you need to seek specialist endocrinologist help for this

p.s i am assuming that you are not a diabetic on medication or insulin?? cause thats the commonest cause of hypoglycemia
