Your dentist will recommend 3 months, but it depends on how you brush your teeth. I used to use a hard bristle (I used to smoke) and over the years did a lot of damage to gums and the removal of enamal from my teeth, which cost a lot to repair. Now, I use a soft bristle, but spend a little more time cleaning my teeth.

every 6 months. unless the bristle's start bending outwards before then.
i use a baby toothbrush to brush my teeth. much gentler, and easier to use and to reach everywhere in my mouth. i'm fairly sure the use of an electric toothbrush wore the enamel down on my teeth, or weakened the enamel. i used one for about a month, and then my teeth started suddenly staining when they didn't before. very upsetting. my teeth also became sensitive. i'm not saying this will happen to everyone, or that it is 100 percent definate it was me using the electric toothbrush; but my teeth staining at the same time of its use points in that direction.

The texts say every 3 months. but if your toothbrush`s bristles ruined sooner that this time, it means that you brush so hard and it could be harmful for your gums & if the bristles didn`t ruin in 3 months, it means that you don`t clean your teeth correctly.

When the bristles start to 'lean' outwards. It depends on how vigorously you brush your teeth. I change mine every couple of months as I use a soft one and they wear down quicker than the others.

My dentist always said every three months unless you get sick with a cold or flu or.... then change it after your illness too otherwise you risk getting sick again.
