There are several reasons that the ankle could still be hurting him. Almost all of them require a doctor to make a diagnosis and start appropriate treatment. If you've seen a doctor and are not making improvement then get a second opinion from a sports medicine doctor, an orthopedic surgeon or an osteopath.

Here are some examples

1-The injury could have caused reactive arthritis. This could be treated with local skin creams, anti-inflammatory pills, physical therapy and/or steroid injections

2-He may have a stress fracture. This needs to be immobilized (i.e. put in a cast or similar device) and pain can be controlled with anti-inflammatory pills and pain medications

3-He may have a tendinopathy.

4-He may have reflex sympathetic dystrophy.

5-He may have tarsal tunnel syndrome

6-He may have plantar fasciitis.

7-He may have a syndesmosis tear.

This is only a partial list of possible causes. He really should see a doctor to get a definite diagnosis and treatment plan.

I fell once and sprained my ankle...well it actually turned out to be a fracture AND torn ligaments....they could tell right away with the torn ligaments because my ankle wasn't "tight"...it flip flopped....so a cast forever and surgery to staple my ligaments together...THEN years later I always complained that it felt like i was being pinched there...so i went to a nerve specialist who was an orthopedic too...well she went back in and said that the orginal MD had stapled them wrong and i had a lump of nerves that were all bunched up and stapled wrong....oh yeah..can you say FUN! so if it's been years...that's not right....i mean who would wait for years to have something heal?! he needs to see and Ortho MD STAT!!...oh and those alphabet movements with the ankle would be good AFTER an assessment!

Definitely x-rays. I was telling a medical professional about my ankle and she basically said I was stupid not to get it x-rayed and checked out by a doctor. She said she did the same thing and five years later when she got an x-ray, she had a hairline fracture and had to wear a cast for a few months. The fracture would not heal without pressure.

you should have him to the exercise where you have to spell out the whole alphabet in capital letters with your foot. when i sprained my ankle, i did that exercise everyday and that helped regain my strength a lot. also, raise up on the balls of your feet for 20 seconds or so multiple times. you can also get a towel and wrap it around the middle of your foot, and then pull the towel towards you (so the top part of your foot comes towards you) and hold it for a few seconds.

i did those exercises and they helped me a lot. i also tried to walk as much as i could on a smooth surface (like the sidewalk, not grass!), and that helped a lot too.

Probably some kind of antiinflammatory like ibuprofen for the joint stiffness.

one of the best things for feet exercise i found is walking in deep sand. it works your foot in every direction strengthening it.
Try not to stand long on concrete or steel floors.

so go on a perminent vacation to a sandy area like a beach.
