A minimum of 3 (for a pyramid of two rows - the mimimum for a shape that could be called a pyramid). For each additional row, you need one more person than the row immediately above. So for 3 rows you need 6 people (1+2+3), for 4 rows 10 (1+2+3+4), 5 rows needs 15 (1+2+3+4+5), 6 rows need 21 (1+2+3+4+5+6) etc. There is a limit to the number of rows you would physically be able to add. This is because each extra row would increase the weight that needs to be supported by the bottom row. The bones in the body of someone in the bottom row would break at some point and so the pyramid would collapse. Also each extra row would decrease the stability of the pyramid and it would probably collapse long before anyones bones broke (how difficult is it to build very large pryamids out of cards?)

The answer to the first bit is one, or possibly three if you want to be strict. The second bit -how big- well, I've seen tv adverts using loads of people, possibly 10 rows. If you are talking about cash pyramids - I wish I'd had the motivation to find out, but when everyone laughed at me for trying to find out how I would have to document it for the inland revenue (saying I'd make nothing) I didn't pursue it.
