What I would love it if a man when saying goodbye would not try and dive in for the first kiss on the first date. I would swoon if he gave me a hug, and kissed me on the forehead and tell me he'll call me soon (if the date went well)
As for the other do's...
Absolutely open the car door for her. This is a MUST for me. I know this sounds silly and old fashioned but I still believe in being treated like a lady.
DON'T keep the ringer on, on your cell phone. Silence it for the night if you have to bring it with you. If there is an emergency, hopefully she'll understand.
Just act like you are VERY interested in her and treat her like a lady.

Do arrive on time
Do make sure your are well-groomed.
Do not spend the whole evening talking about yourself.
Do not try to force your date to do something she doesn't want to.
Do not try to rush into anything, let things go at their own pace.
Do be courteous to her friends/family (especially her dad), if applicable.
Do treat her with as much respect as you would hope someone would give your own sister.

My moms friend gave me some books and one of them was the idiots guide to dating. It said that you can't have sex, but you can give someone a rim job, which to me, seems almost as intimate as sex, if not more.
