You can put anbesol on it to ease the pan some, but the first aid would be to get very sharp wire cutters and clip off the barb of the hook, and pull the rest of it out.. It will require 2 people to do this..so, you will need to recruit help.. Call friends to find a pair of wire cutters if you don't have them..Most households do..
After pushing it through, disinfect with beta-dyne..

You should take the puppy to the vet and make arrangements with the vet to pay the bill. The dog at the least will probably need a tetnus shot. If you feel really froggy, you could get a pair of wire clippers and cut the end with the hook and slide it out the other way. But the dog will still need a shot, as chances are, the hook is old, probably somewhat rusty and God knows your boyfriend didn't wash it in antibacterial soap before the puppy bit into it. I hope the puppy is okay, call the vet now!

It is in the lip so there is no need to rush to the vet. It is very painful for the poor puppy, but you can give the pup some benadryl to make the pup sleepy and then take wire cutters and clip it into. If the barb is all through the lip, clip above the barb and pull through.
This will take two people but if it is sleepy it will not be so bad.

Since Vet money is an Issue that I know well with 3 dogs and 5 cats Just get a small pair of wire cutters cut the hook end off feed back through the lip and clean well wth peroxide and an antiobiotic lotion for 5 days or till well healed

Clip the barb off with wire cutters or even a strong pair of gardening hand shears (rose cutters, small branch cutter).
Once you cut the barb (hook part) off, pull the the the other end out. Wash it ASAP with some Hydrogen peroxide and watch for yellow discharge. If you notice massive swelling and discharge you might have to take your pup to the vet to get antibiotics.

Take her to the vet IMMEDIATELY!!!
I understand needing to save money - But you would rather have it done right and painlessly rather than risk the chance of major pain and/or a horrible infection.
Plus, it's not a major operation or anything. I doubt it will cost that much at the vet. Probably the price of a regular visit (between $35 and $75) + emergency visit cost (between $75-$100 depending on where you live) + the cost of antibiotics (probably less than $50).
Maybe you could even call your local humane society. One of their vet tech employees (I'm sure they have 1 or 2) may be able to help you out as well for much less money!

TAKE HER TO THE VET! I also do not always have the money to go to the vet when something goes wrong, but if it was my child I wouldn't hesitate to take him/her to the hospital so why should I treat my dogs any differently? You can cause major damage if you try it yourself!!
The vet may also work with you on a payment plan if the price gets too high.

If you don't take her to the vet you could be arrested for animal cruelty. I think I'd rather spend money than time in jail.
Also, how would you feel if you had a fishhook through you lip? You would probably want to go the doctor right? The vet would be the best thing to do.

Money should be the last thing on your mind. This isn't even your dog!!! You NEED to take her to the vet. They can get it out. They may need to sedate her, and you will definately need another person or two to hold her in place. They will probably give her steroids for the swelling and some kind of anti biotic for infection. I'm sure your boyfriend won't be upset if you do this. If he even cares remotely about this animal he would do the same thing himself.

Wire cutters. Snip off the end so you can pull it the rest of the way through. Neosporin on the wound till it heals.
Actually had one in the program at about a year old do the same thing. We called her our punk rock dog. She was in a foster home an hour away from the nearest store let alone vet.
