If you have a stomach flu and you are vomiting a lot, it's possible to vomit bile when you don't have anything else in your stomach.

If you don't have food in your stomach, you can end up vomiting bile until you end up vomiting blood from irritation.

My son is a Celiac that vomits when he has gluten. Once we know he has no food in his stomach and he starts vomiting bile, we make him drink water between each evacuation so his stomach will not get to the point of bleeding.

when someone is sick and is unable to keep food or liquids down the body's intestinal track pushes the bile up from the muscles being put into the upward motion. This is what some may call a dry heaves when the body has nothing else left to bring up that is put down normally.
