Is the bulge near the dog's rectum? If it is, it sounds like it could be an impacted anal gland. Sometimes they rupture on their own if they become abscessed and they leave a small opening near the rectum that has to be cleaned and sometimes, sutured. If the "bulge" is a tumor more invasive surgery may needed.

If it is just an impacted anal gland, the surgery can be preformed with just a light muscle relaxant such as Xylazine (Rompum) and a local anesthetic (Lidocaine)

Under no circumstance would I choose a general anesthetic if your dog have poor kidney function. Hopefully the vet has your dog on a low sodium, prescription diet for his kidney problem.

it sounds like a cyst or blocked anal gland and yes they can be removed under a local anaesthetic quite safely they only have to make a small incision to drain it off
