Memory and concentration is just like a muscle - not quite permanent, but lasting results can be achieved.
To remember names, try the following. When introduced to a new person, pay attention, repeat his name out loud. Then, 1 minute later, either repeat it in your mind what his name was, or call him by his name in conversation. Then repeat his name in your mind like 5 minutes later. Then do it again 30 minutes later. You will never forget this person's name again.
As for reading - try reading something very very interesting.

Too keep my mental agility and memory in check when I was off a semester during school I would do these activities: I would pick a random card from a deck of cards. Try to remember it throughout the whole day, right before bed I would see if I rememebred the card. Another activity was the memory game. I know it sounds lame, but use a deck of cards and try to match cards up and time youself. Try to get faster each week. These are just my suggestions. Good luck

Reading just takes practice. Find a subject or genre the interests you and read more of it. Ask your friends what books they read for fun, then discuss it with them. You can also play games that will help your memory. There's a new Gamboy DS game called Brain Age that is exactly for that purpose, but there's many more out there.

Practice. One of the most famous researchers of memory (Ebbinghaus) just increased memory through constant practice... to a limit, of course.
Also, use mnemonics and other tricks to help your brain make connections to things you already know/remember.

start small. that's what I had to do. I couldn't remember names to save my life. you fix it by doing this: when you meet some one, always say their name back to them. example. (and you have to look at them when you do this.) Hi my name is Bill. and you say always with the name..... it's nice to meet you Bill. if you say the name while looking at the face you'll have an uncanny rememberance of their name. it takes some conscience effort at first, but will become second nature before you know it. start there and then your mind will recognize the importance of remembering things you want it to.
