You first have to define what an automobile is because they didn't just appear all of the sudden. They evolved from more simple contraptions one component at a time.
Next you have to define what an accident is because the first drivers may have driven a few hunderd yards and ran into a brick wall and then call their experiment and journey a resounding success because the investors were lining up at that pile of scrap handing him money for one just like it.
Finally you have to identify who determines if an incident is an accident: you today or the driver of the "automobile" back then
Then there are those who would consider the automobile an accident and therefore there is no "first" because this is the "first".
