The phrase "When Christ and his saints slept" was used to describe a particularly bad time in medieval English history, when two people fought for the right to claim the English throne.
Empress Maude's father, King Henry I, willed the throne to her. Except that English nobles wouldn't bow to a woman, so they declared one of their own, her cousin Stephen, king.
Stephen's and Maude's armies fought for decades, and of course it was bad all around for England, which inspired that description.

There wasn't any. True anarchy cannot exist in any sort of society. There will always be someone on the higher end of things whose responsability it is to make rules. Even if they're not enforced, the rules still exist, and anarchy simply becomes another form of law-breaking.
