As far as I know there is no best way to ask. But if you feel that you are really good and your boss would not let you go, do the following.
Start doing job search on the company internal job sites. Make sure your Boss watches it before you hide it from your screen. Keep some printouts at your desk for the job requirements. And look very sad. Within couple of days your boss will ask what's up. And then you can say. Mostly the results are positive.

Tell him/her that you have been working for the company X number of years and have given 110%, worked long hours, arrive on time, etc...and that you're wondering if a pay increase would be possible since you have proven yourself worthy
