An employer can not choose a set of an employees they would like to cover for health insurance in any state. All "eligible" employees must have the choice to have coverage or not. Eligible employees must be full time (25+ hours a week), the employee might waive coverage if they are covered elsewhere (i.e. under their spouses plan).
The only way to pick and choose which employees you want to be covered are under non-qualified life insurance plans.

It depends on your state employment law and how many hours the employees are working. You need to do more research and have a more specific question, but also research with the professionals in the field.

Of course they can. It's just a benefit like any other. There's no law that compels an employer to offer medical insurance to ANY employee.
