Unless someone has sat down with the sheet music and deciphered the chords out of the melody lines, I think you would have to do it yourself.

I know the first chord is C sharp minor! And the first note is a G sharp octave in the bass cleff.

I imagine the piece is written for the piano. You can figure what chords are used by determining at least three of the notes, either in a chord pattern, or given in succession.

You'll have at least four sharps to work with, and the common chords would be C#m, G#, Dmaj6, A, and B7. Just a guess here since I haven't seen the composition.
