It's mostly a greeting when someone says it to you. The most important thing to know is that it is NOT a question. They are not asking 'Are you alright?' - it's just a way of saying 'hi'.
If someone asks 'how are you' though, you may reply 'alright thanks'.

Treat it how you like. Alright pretty much means hey. Depends on how you say it of course, if you are ask it in the tone of a question then just reply. If someone just casually says it to you, then don't give them a lecture about whether you are in fact alright or not, yeh!?
