Most Motor Factors will do it for you. In England & Wales you'll need to show your V5, but in Scotland you can just turn up and ask for HRH 1 with no ID if you like.

Take your V5 document to a garage or car parts shop eg Motormania / Halfords (to prove the identity of the vehicle and also that you own it) and ask them to make one up for you, it takes about 10 minutes.
You can get more customised ones but they are made to order, take longer and cost more
