God's blessings flow regardless of someone saying it or not. So they are actually being polite and expressing a note of compassion at a sneeze...but it does not direct God in any way.

I think that it depends on what it's said for. Some might say, " God bless you." and mean "F' you." While others might mean it wholeheartedly. God will bless people for saying that, or being the object of that, if it's in His will.
