He was born Cassius Clay, and under that name he won a gold medal at the 1960 Olympics in Rome. After claiming his first title by defeating Sonny Liston in 1964, Clay joined the Nation of Islam and changed his name to Muhammad Ali.
Why? Because he wanted his name to be more in line with his new faith in Islam.

Sorry, but it was well after 1959 when he changed his name to Muhammed Ali. He won the heavyweight championship in 1964 as Cassius Clay.
