Absolutely!! Your posture affects your back and neck, which are with you for a lifetime ... You should try to think of there being a strong string attached to the top of your head, and that someone way up in the sky is pulling on that string.. Think of this often!!
Also, your ears should be aligned with your shoulders so that your neck muscles are not having to work at holding up this heavy 'bowling ball' out in space. So, move your head back in (like a turtle!) to where your ears are above your shoulders..

Yes! Poor posture can lead to any number of back problems, knee problems, foot problems, and cause stress on the body that can result in any number of chronic disorders.

I read something a while back that said studies showed that sitting too straight could actually be stressful on the spine. I am sure slouching too can be as stressful. I think that the best posture is one that you are comfortable with. Sit straight but not ramrod straight. Make sure you are not bending your neck too much in an uncomfortable position either. If you are finding yourself slouching too much as you read see an eye doctor to make sure your sight is okay.
