Yes the alcohol evaporates during cooking to leave the actual flavor of the beverage in the food, cook on a low heat as the alcohol content can cause a burn if left to simmer on a high heat.

Generally, yes it does.
The exceptions depend on the amount of alcohol is in what you're adding. Brandy and rum are obviously higher in alcohol than beer or wine. It also depends on how long after adding the alcohol that it remains cooking. If you're doing a quick flambe' for effect, then extinguishing the flames and serving immediately, then yes, there will be alchohol remaining. If you're doing a wine reduction and the wine has been simmering for an hour, there isn't a drop of alcohol left, I guarantee.
Consider the two factors and you'll be able to figure it out with each instance.

The alcohol never completely cooks off those that for religious reasons don't partake of alcohol shouldn't cook with alcohol.
Or if you are allergic to alcohol, you shouldn't use it.
However the remaining amount is so insignificant, less that vanilla extract, mouthwash, gripe-water or other household products.

Yes, so it's impossible for the kids the get drunk! It also adds tons of flavoring and I would definitely recommend it. Martha Stewart is practically alcoholic because she always has alcohol around for her cooking.

the alcohol does indeed evaporate and leaves behind the flavor...and it is a good show for a flombay (setting a sauce or a dish w/ alcohol on fire) pretty redish pink and blue flames!

yes. since the boiling point of alcohol is lower than water, the alcohol burns and evaporates first, before the rest of the sauce.

Yeah, when my kids were little they would pretend to be drunk if they ate anything that had alcohol cooked into it. They would be quite silly.
