hey Dear,write your current address you need not to write your History now.if they ask in then give.they will get you if you write current address so write in bracket (Current Address)

You can technically consider your zip code to be the same as your city but post office workers prefer to see the city or town name on the envelope. For this reason, I would recommend using the name of the town because you might just find your mail returned to you some of the time.
