I can't hear myself think with my ears, unless I am talking out loud. I think if somone says that, it's a figure of speech. If you have multiple personalities, maybe you can.. but I don't think you'd know you were hearing your self think...... Unless.... you knew you had split personalities, and you carry on conversations with yourself. I guess you'd never feel lonley...

In the same way that you can't hear yourself think when there are too many distractions around you.
Right now I'm trying to compose these words by saying them to myself in my head. It is nothing audio, i.e. nothing passes through my ears and then to my brain, but rather my conscious thoughts which I have to organise by "voicing" them to myself so that they make sense to me.
