Make sure that your kitchen is warm enough and free from draughs, is your yeast quite new?, make sure the water is at the correct temperature.
My wife and I started to make bread after I bought her a bread maker, and I started by reading the instructions and recepie and up till now all my loafs have turned out fine, I have never had a loaf fail to rise etc.
One other thing and that is, make sure you use level spoon fool of ingredients when asked to do so. Stick exactly for now to what the booklet says and things will turn out fine.
Basically it's either the yeast has gone off, wrong amounts of ingredients, water temp too hot / cold, room temp too low or draughty.
Stick to the booklet provided word by word and things should turn out fine, another thing is, to use the strongest flower you can get, and a local baker told me to use fresh yeast which will make the bread taste better.
Hope this helps, although I'm no master at this but all my loaves have turned out fine and all I did was read the booklet and stick faithfully to what it says.
Oh sorry I forgot to mention, make sure that you put the engredients in at the correct time, don't put something in before you should have put something else in before that as It will make a difference between success or failure.

Check the expiration date of the mixture/ yeast. It can go bad and your bread wont rise. If the date is ok, you may want to check manufacterer information and see if there is a warranty, or call information to ask them if there may be something wrong with the bread maker.

It's not a Cookworks product is it I had one and the bread was raw on the inside and very pale and it had been in the bread maker for ages, try a more expensive brand like kenwood or Morphy Richards that's what i am gonna do next time i buy one, if i buy one.

I had the same problem the other day. I used instant rise yeast instead of bread machine yeast. It will make a difference!

make sure you're using bread machine yeast, make sure the waters cold and the ingredients are put in in the order the manufacturer suggests. if the waterrs too hot then it over proves the yeast and it wont rise. thats the most common mistake

if you're using a whole wheat or whole grain flour, you need extra yeast and maybe even a lil baking powder to make the dough rise. That's because the protein (gluten) in the wheat bread gets stronger as it is kneaded. you should give the dough some extra time to rise.

if you're using white flour, then I don't know what the trouble is.

Is your yeast in date and are you adding the correct amount. If you are using prepared bread mix then perhaps you are not baking at the correct program, you could also try a teaspoon of sugar to your ingredients that sometimes works.

If you lower the bread-maker itself, it will achieve the same result.

Too much or too little water could be your problem. Also try using fresh yeast instead of packet dried yeast, that worked for me

If you're sure you added yeast, it could be that the yeast is too old or that the water you added was not quite the right temperature.
