it depends on what state you are from. call your local worker's comp. commission office and they will tell you.

You can ask a judge to rule on that by filing an objection to the next one they schedule. Or the current one scheduled, if it's not already past. A judge may find they have a valid reason for wanting it, but it shouldn't hurt to ask. If you are claiming a permanent disability, they have a right to know your progress. Every three months would seem to be reasonable. If your condition has not changed significantly, and the doctor is willing to say it's not likely to change, then perhaps less often would be reasonable. The judge will decide. They tend to err on the side of caution, but talk it over with your lawyer at least.

Not sure how many really. I even work for work comp defense as a support staff. How many have you attended. I am sure they can get a couple of opinions.
