She might have a stomach infection even if she looks healthy, or maybe she is eating something in your yard that you are not aware of (lizards, birds, mice) that can cause bad breath. There are some special treats that contain breath fresheners, you can buy them at pet stores.

Maybe it's the cat food you are giving her.
Sorry, but I have to go here, you should brush her teeth. I know it's gross, but I had a cat and when she was about 10 we had to have several of them removed because they were rotten and she was drooling all over. The vet said it could have been avoided. Talk to your vet, because you should not use regular tooth paste and toothbrush.

you really should brush your cats teeth about once a week. take your cat into the vets and they will clean your cat's teeth.
