Lots have touched on it but none have offered the correct solution yet. I cannot eat pineapple because of the high acid content HOWEVER if you place the pineapple pieces in a bowl of cold water with a very small amount of vinegar (like a small squirt in 2 cups of water), let it soak for 15 mins or so the acid will neutralize and then you will be ok. Don't poo poo until you try it, I'm back to eating pineapple with no problems.

What ever you do -- don't ask a doctor.
Especially, don't ask an allergist.
Take import medical advice from random Yahoo users.
Oh alright -- they're canker sores.
Canker Sore Causes
No one knows what causes canker sores. The vast majority of people who develop canker sores do not have another problem as the cause.
* The disease is suspected to result from a reaction of the immune system.
* Additional factors may include the following:
o Bacterial infections
o Hormonal disorders
o Stress
o Trauma
o Heredity
o Food allergies or sensitivities (acidic foods such as pineapple)

It's from the acids in the pineapples. The 'sugarpines' from the Caribbean are small really sweet pineapples but without the acid have a texture like custard. Too bad they don't ship very well.

Yes try adding a bit of salt to the pineapple to reduce the acidity. it won't have much change in flavor. I get this from tomatoes, citrus pineapple and juices, the salt helps reduce the acidity, or at least it reduces the impact the acid has on my tongue. I won't pretend to know what it does to the acidity since I am not a chemist

I'm a dentist.
It could be the sheer acidity of pineapple irritating his mucosa. It could also be an allergic reaction to the pineapple. Sometimes these allergies manifest themselves as generalized redness and irritation, and at other times as ulcers (perhaps it can spawn apthous ulcers).

They're probably small ulcers. Pineapples are very acidic and can cause this. He should not eat pineapple. It happens to lots of people.

It's a food allergy, I heard on the radio the other day about other people with the same thing. I dont know if it's treatable, it may be through de-sensitization injections, speak to your doctor... De sensitization injections are a series of injections of the thing your allergic usually twice a week for about 3 months, gradually increasing the dose, your body becomes immune to it.
