On the invitation itself, the wording will help them to know the formality of the event.
First names only (John and Jane) will let them know that this is a casual to informal wedding.
Full names (John and Jane Smith) tells them that this is an informal to cocktail wedding.
Full names plus titles (Mr. & Mrs. John Smith) tells them that it is a formal to black tie wedding.
You should know how formal of a wedding you are having by the time the invitations go out, then use the wording to match. That plus the time and location will tell them how to dress for the setting. It saves you the headache of trying to put the dress code on the invitation, and it will limit the calls about attire after they recieve them.

Of course you do. Unless you're inviting random strangers to your wedding.
What a strange question.

On the outter envelope, of course you do. On the inside envelope, it would be optional. If I was inviting say my boss and I called him Mr. Smith, then I would not put John and Mary on the inner envelope though.

yes you do. There could be more than 1 John especially where seating is concern. It also gives a sort of recognition. Go ahead put their last, save confusion

Yes. They are important in your life, or else they would not be there right? So you respect them and therefore publish their names along with your own. Congratulations to the soon-to-be-spouse!!
