I was at a chamber music concert once, very small and intimate, and many people applauded after the first movement. And the second, and so on. When they had finished the entire piece, everyone hesitated to clap. It was quite hilarious....

I've been around people who have done it. If I'm going to a concert with someone I know has never been to one before, I tell them not to clap. Most concert goers aren't bothered by it. I'm guessing if the person just stops when he/she realizes the mistake and then waits to clap until everyone else is that will be fine.

Yes. At several concerts. Most of us attend concerts by our own favorite performer or very famous performer to enjoy. And just on the appearance of the performer on the stage itself people clap and give standing ovation. Why.
Some times I have given the lead others forget and to follow me. This is is also sort of Etiquette.

depends on the kind of concert you're talking about. If it's a rock concert, people don't really clap. they just listen or mosh or whatever.

Yes, at my daughters school concert in the hope that they would take a hint and immediately play the next piece ( hopefully not as screechingly)

my mother did that at a concert in Austria. Then proceeded to ask if it was over and left the room. The whole room turned to look at the dumb American.
