Ringworm is a fungal infection (much like athlete's foot in humans) not an actual worm. She probably picked it up from the shelter.
As for keeping her in a crate, it is not a bad idea. Ringworm can be spread to humans. Keeping her in the crate would keep the fungus in one area until treatment is done.
It won't hurt her. She may whine a little at first but alot of dog trainers recommend crate training anyway.

As for everything she has been treated for, all puppies are born with certain parasites. The shelter was unable to treat before you adopted her because she was too young. However, they should have mentioned it to you beforehand. Same goes for the fleas. Most flea meds can't be used at that age.

Good luck and hope everything goes good from now on.

First off, thank you for picking a puppy from an animal shelter. Secondly, I'm sorry to hear about all of the trouble the puppy is going through. There is no quick solution to this answer. Just keep diligent with what the doctor has prescribed and give her all the love and support that you can. My girlfriend had ringworm that she thinks she got from her kitten. That could be one of the reasons why your vet is telling you to keep her crated. So be careful yourself. It's in the best interest of the dog, you're not being a mean pet owner. They cant judge whats best for them. Just be persistent with your treatments and patient. With time, the parasites will go away and you will get to have fun with your new puppy.
Good luck!

trust your vet and don't let her wonder the parasite can live in carpet or ground or anything else she comes in contact with...keep her on sentinel it does work...
