You can write to the department of vital statistics in the state or province where you were born, and request a copy of your birth certificate. You'll have to give them your name and date of birth. There is a small fee for this, but you'll need it if you want to get a passport.

Contact the State Department of Vital Records for the state that you were born in. They will send you a form to fill out. There is a small fee to obtain a copy but I don't think it is more than 20-25 dollars.

Depending whether or not you were born in the USA the other answers are good. If you were not, you need to find out what the rules for getting your birth certificate in your native county. (If one was filed.)

The best way to find out about your birth certificate is to ask your parents, or a relative old enough to remember your birth.

Also, remember the new copy of the birth certificate may not contain the time you were born. (Mine didn't.)

It depends on where you where born, I was born in Oregon state and my birth cert was issued by the Oregon state Health Divison, Center for Health stats. Try the website www.vitalchek.com

go to the county courthouse where you were born and usually have to pay a small fee like $10.00 or so and they should give you one
