On the envelope you address it as:

To the Countess of Bradford
Bradford House
Old what ever the village is called

In the letter you are writing

It Dear Lady Bradford,

If you are on unfamiliar terms, call her "Your Grace". If you are intimates Lady Such and Such will do until she tells you other wise
