From time to time, I kind of hack up these little (and sometimes not so little) clumps. Theyre kind of white/yellowish, and smell terrible! Sometimes I can see them way in the back of my throat.
I brush and floss 3 times a day, and gargle once in the morning.
What are they and how do they happen?

Quite a few posts but no one said what your condition is. You have tonsil stones. Just like one who has gall stones or kidney stones.

Check out this link. The fix is to get your tonsils out.

Just wanted to say I have had this same question and have wondered what these things are. It's been awhile since I've seen any, but you wonder when it happens just how long it's been there at the back of your throat and how long your breath might've been stinking. Plaque sounds like an accurate answer . ..it really is about the most horrid smell there ever was . ..I'm going to guess there is some kind of gland back there that secretes it; or some kind of fold of flesh plaque gets stuck under from food passing over it and then it not getting rinsed away

They become from your tonsils.
The tonsils works as a filter for your throat and somehow breathing system.. The stuff you spit are produce when your tonsils are sick, whether it can be with a disease to your throat, some pleople also get that with the flew.

In summary, they are the effect when your filter is sick or too dirty.
The smell is a result of all the dirt and bio-chemistry that your own body performs to heal himself. You don't need to take your tonsils out, just find the right medical treatment.

One last thing, never smoke! It is terrible for your tonsils, even if you are around people who smokes.

It is old food trapped in the craters of your tonsils! The smell is just that of decaying food! Either have your tonsils removed or gargle with warm salt water to loosen them up. They are more noticable this time of year because of weather changing and people are starting to get colds and allergies! They are very common! No worries though!

When you breathe you have dust particles enter your mouth/nose and go down your trachea or wind pipe. Rather than these enter the fine intricate alveolie air sacs and cause breathing problems they must be bought back up. This is done by tiny hairs and a lot of mucus lining the trachea. This is bought all the way up to the mouth and back of throat to be swallowed back down the oesophagus to the stomach. Obviously yours are being coughed up and spat out rather than just swallowed down. You may find they are lumpy because you are working or living in a particularly dusty area.

Well actually that means you probably have really big tonsils....Those two things in the back of your throat on each side. This has happened to me before and i freaked out and asked my dentist and he told me that they are actually little pieces of food that get stuck in your tonsils, he told me the best way to remove them was to gargle peroxide water n salt all mixed together because it looses them up. My tonsils have sinced been removed and it doesnt happen anymore :) Lots of people get this!!!

I forget what they are called but they come from your tonsils. There are little sacs in the back of your throat, and I had my Doc tell me it was dead white blood cells. I kept getting strep throat and colds etc. I finally had to get my tonsils out, and I've never had the problem again. If you get sick alot or at least alot of colds/sore throats, I would go to an E.N.T. doc. (ear/nose/throat)

Perhaps you've been sleep walking and killing animal out in the night and eating them raw, which would mean that what you're coughing up is half-digested bits of animals bones.

I hope someone on here really knows - I have & hate this, too. It is worse around this time of year - when allergies or a cold is getting me. Nasty!
