A personal trainer does a tailor made exercise programme just for you, depending on you abilities and your goals. They then train with you to make sure you are doing all the exercises correctly and to push you that little bit extra.
How often you train with your personal trainer is up to each person and their bank balance. I train with mine once a week and then I train on my own 4 times a week.
A personal trainer also keep updating and changing your programme as go, usually every 4-6 weeks

A personal trainer is a person that you have just for your session at the gym or at your home (i think) and they pretty much exercise with you, teach you good ways of excersing, etc.
