My buddy claims that she heard this in a work environment comprised of a lot of ex-military/navy workers. Her usage of it was in the sentence, "I failed open today at 5:30am," meaning that she woke up at that time. Has anyone heard of this phrase?

That phrase pertains typically to a piece of equipment like a valve, for safety aspect or reason. It might be better that when a power source fails, it fails in the open position. Like wise a failed closed position , would be a safer condition in case its power source failed. They probably at some time adopted the phrase.

It could mean that she failed to arrive on time at 5:30am or failed a surgery or something related to her job...sounds something like that anyway. It seems to relate to some disappointment.
