how high a fever? what other symptoms does he have? why was he prescribed Unasyn? was that for this illness or a previous one? unasyn isn't just a cure-all for fever. if it's not caused by something that Unasyn would treat, then taking it won't do a lick of good.
but anyways...If the fever is running in the 104 or higher range or if he looks tired and lethargic, then a visit to the ER is probably reasonable. A good way to check if he is dehydrated from fever is to check if he is making tears when he crys. If he is not, then he may be somewhat dehydrated.

Unasyn antibiotic?
How high is the fever? Take him to see a doctor. Try some children's tylenol to help with the fever. If it is unusually, high (over 104 degrees F, 40 degrees C) , something definitely needs to be done. that detail when you ought to be concerned, call a doctor, or go to an emergency room.
Remember, a fever is a natural part of our body ridding itself of infection. So unless it is dangerously high, it's often best to keep an eye on it and let the body do its work.

If the antibiotic has not made any difference within 48 hours, it will not work. What are the other symptoms? If none, it may be viral and this usually takes 5 days. If there is no focus of infection such as ear, throat, lungs, etc. an antibiotic should not be started.
If the temperature is 100 degrees F or more, than give an antipyretic such as tylenol. This may be used after 4 hours, if needed.

Use tylenol to reduce fever.
Sounds like the antibiotic is not working needs taken back in. Keep fever down until you can get him in, (sponge bath) if necessary, push fluids. if skin gets dry or fever to high and won't come down or (listless) take him to the Emergencey at the hospital.

Go to the doctor if the fever is too high... but i give my daughter Motrin and the fever goes away fast and last pretty long...Tylenol did not work for her, it didn't help at ALL with the fever.. Make sure you give your child lots of of liquids and give him lots of baths when the fever is high it can really help

a small child can dehydrate easily with fever
