The answer is based on faith. If you truly believe that Jesus Christ is God, that you are a sinner and need forgiveness, and that the only pathway to be forgiven is by the grace of God through faith in Jesus Christ, and you have asked God to forgive based on your confession of faith in Jesus Christ, you are saved.
(What a run-on sentence!)
In regards to your other question, I believe in the idea of eternal security, or 'once saved, always saved.' I do believe that a person can lose their salvation during the time of the Great Tribulation, when the mark of the beast is instituted; if someone who is/was a Christian takes the mark of the beast, they are damned. This will only apply to the Tribulation Saints, those who come to faith after the rapture, because the mark of the beast will not be around until after the rapture. So, for Christians today, I believe that if you are saved, you are safe.
There is an argument that if you completely turn your back on God and Jesus Christ, renounce your faith by word and deed, and reject the teachings of the Bible then you have renounced the Holy Spirit and have lost salvation.

If someone else is they will say it probably. Usually when you first meet someone if the first thing the say is I'm a Christian then there's a 99% chance they are. But if you've been friends with someone for months and they never mention it and you've had time to chat then they probably aren't or they've fallen away which they still are they just might not be too happy with God

The only way you will know for sure is if you die and you find out in the after life that you are saved. Until then you most likely will not know for sure.
