There is no blacklist or even a national registry for pharmacists, only licensed or not licensed in a specific state. A pharmacist denied a license in one state could, possibly, manage to get licensed in another state.

Go to your state's Board of Pharmacy , and check to see if an individual is licensed, or verified, in your state.
