I think i've seen pin holes in the sides, but maybe they're bubbles, any clue to how the carmel gets inside?

I have seen it done. There is a machine that dispenses the chocolate and carmel almost at the same time. The chocolate release first and then the carmel, they are fed through a tip like on the end of a pastry bag, one tip in front dispenses the chocolate the second tip sits on top of the first tip and out comes the carmel. Think of two pointed ice cream cones sitting together. AS the chocolate comes out it will fill a mold and set the carmel comes out and the chocolate finishes the mold to cover the top.

I'm not sure, but however they do, that's ok with me as long as it gets in there! lol
