Well, are you talking figuratively or literaly?
I think dogs have that 6th sense, they can tell the difference between someone who is a nice person and isn't. I have seen my dog go nuts over someone before because of a certain way they looked..and they turned out to be a real scumbag.
Now if you mean literaly...they were doing a study to have certain dogs be able to actually sniff out certain cancers on a human that were indetectable to doctors testing. That was on the Discovery channel. So maybe they are finding its possible for them to detect a heart problem as well.
Otherwise I would think that a dog can simply sense a change in their owners rythyms, or normal functionality. I think any animal can sense certain changes in the human body. One of the interesting things that just makes them unique. They say it is actually in the smell however, that everything gives off a smell. Sometimes maybe a sick human gives off a different kind of smell.
So which did you mean lol.

Hm, my great-grandmother use to tell me this. I believe pets can tell what kind of a person one is..such as like you said a good heart from a bad heart.

dogs are pretty instinctive... they can tell a lot from just your body language. But if you are referring to a medical condition, only a few dogs have been known to have that capability. They may sense something is wrong, but can't identify it.

dogs go by your body posturing and tone of voice
generally mean people walk and talk alike
but some have masked it well and just have vicious hearts
we call them politicians
