My daughter spends two weeks every summer with her grandparents and we always give them her insurance card, her social security card, a photo ID that we make on our computer with all her stats (thumbprint included), and a letter saying we give them permission to seek medical attention, emergency or otherwise, in the event we cannot be reached. If your mom is staying in your house or if you are in the same city, make sure she is listed in your daughter's file at the pediatrician's as someone who is allowed to bring her in for a visit.

You just need to leave a letter stating this, We, Joe and Jane Doe, give my mother Ms. Judy Doe authorization to make any legal decisions regarding the daily life and health issues and decsions of our daughter, Jamie Doe during September 14, 2006 through September 30, 2006 while we are away on a trip. And then sign it in front of a notary. (You don't have to have it notarized but is a good idea to do so). Obviously need to make sure you leave any pertinent information so that doctor or healthcare provider can reach you. Also make sure you give her the insurance card and shot/medical records you have. Have fun on your trip.

I have done this before. What I would start with is a letter saying you give her authorization to take your child to the ER and then have it notarized. Also I would leave your mom a list of her doctors and medications she is allergic to. Leave as much information as possible with her so that they can reach you in case of an emergency. To be honest regardless of how much information you leave you will still worry but the vacation is worth every dime. Have fun!!! and try not to worry she has done this before and you turned out OK.

Include a list with all the numbers you and your husband can be reached at. The pediatricians phone number. Also dont forget to leave your daughters insurance card. You will need to have something signed by both you and your husband enabling your mother to make decisions regarding your daughters health in your absence.

Unless your leaving the country or can't be reached by phone wherever it is your going ya don't really need to leave anything except a phone number for the family doctor and maybe the insurance info she's the grandmother she has permission to take her to the doctor they might just have to call you to confirm information I take my neighbors kids to doctors all the time they don't even ask how I'm related

I suggest you call your medical insurance provider to find out what is needed. Because even though YOU may give your mother authorization there might be a "clause" in your insurance papers that says the insurance company doesn't HAVE to pay for services rendered and you may wind up coming home to a pile of doctor bills.

Yes in order for you parents to seek medical treatment you need to leave a letter stating they can authorize treatment. I would also leave a note with the regular health care provider. Make sure you leave the insurance card also. If she will be attending daycare make sure they have info on hand and available. Make sure you write down if she has any fears or allergys as reminders for your parents. I wouldn't expect gram and gramp to follow your rules as to bed time, and other stuff (they tend to spoil as the please!) Enjoy your trip.

You need her to have the insurance cards and a letter stating that she is the guardian. Give her all the information and documaentaion of shot records, allergies, ect. Have fun on the trip

Most hopitals have a paper you can get that you can fill out just in case of an accident. The form lets you give your mom permission to have your child treated by the er or your childs family doctor.. I gave one to my mother in law just incase something happened to my child while she was watching them. At the hospital that I go to they have to be renewed every six months. The paper allows my mother in law to take my child to her regular doctor in case she is sick or to the er if something worse happens I suggest that you go to your doctors office and ask them if they have a consent form .
