Because you posted in Health I assume your reference to getting medical aid in case of emergencies. It is now or never and makes the difference between life and death. For heart it is one hour after which there will be irreparable damage to the tissue. So is the case with infections, the longer you postpone treatment the more medicines you have to take. They say, a stitch in time saves nine. Time is of essence and it is time I caught up with my sleep.

my wife says it means that it is urgent eg if someone was in a accident "time is of the essence" which means that you would have to help that person straight away or they might die.
