If you mean the card that comes in an invitation, you should mark "no" and return it, It's nice to send a card or note saying that you regret you will be unable to attend and wishing the couple well,.

How much you care about them, and you're sorry you are unable to attend. About a paragraph of sentimental thoughts and such, wishing them happiness.

Actually it would be most proper to send a real congratulations letter, not a greeting card. But I suppose you could write a letter within the card.

On the left side of the card in the blank space write something like:

Dear XXX and XXX,

I'm terribly sorry that I am unable to attend your special day - I'm sure it is going to be a beautiful day!

I wish the both of you a lifetime of happiness, as you both deserve it and make a wonderful couple! I only hope we can make plans to see each other very soon.

Have a beautiful day!

Congratulations on your wedding. I wouldn't include the reason or excuse for why you couldn't make the wedding. Just leave it at Congrats.
