Munch had a lot of emotional problems to deal with, to say the least, including his love/hate/fear of women, the death of his sister at an early age, overly strict parents, etc. He himself had several breakdowns.

It is more likely that the figure on the bridge represents himself.

I read somewhere that it's supposed to be a friend of his who was having a nervous breakdown. The sky suggests it happened after Mt. Krakatoa blew. I'll try to find it.

Okay, I was wrong. The friends were the two people walking behind him. He was the one with the panic attack.
