If you are sending the fax to an office in a very large corporation where someone will pick it up and deliver it to her... then you do not need a cover sheet...

If it is to her personal computer, then you might want to put in several pages before your notice.

??? This is odd. You're asking a question regarding fax etiquette, yet the whole thing is just plain rude.

If you need to tell someone something important, then you tell them IN PERSON.
If it isn't possible to physically meet the person within a suitable timeframe, then you PHONE that person. Writing a letter, or sending a fax does not allow the recipient any kind of response. In fact, it's not only rude, but cowardly.

No you just need to act like a man or better yet, a human being with respect and tell her in person. Phone calls, faxes, emails and text messages are insulting but atleast she has something to look forward to. She can find herself a real person to date once you're out of the picture.
