There are 2 reasons to have 5 days off work that come to mind and as you are only taking 5 day off work you do not need a doctors note as that is only required for 7 days or longer.
The first reason is gastroenteritis as this would require you to have minimal contact with others and can last over 5 days
The second is that there has been a death in the family in another part of the country.
People are not really willing to go into much detail for either reason.
They have worked for me and my friends.

Thank you for taking the time to read this note, but I am suffering from a mental breakdown, my doctor assures me that I should be OK after a few days total rest and this should be taken immediately.
I realise that this will cause extreme difficulties within the company, but I am sure that these will be smoothed over by the person you hire to replace me. I accept that you feel, quite justifiably, that I am a lazy wastrel who would prefer life on the dole, sponging off the state and the sooner you rectify the fact that I am employed by you, the sooner I can apply for any benefit that I am not entitled to but I will be claiming,

Say that you've had an acute episode of "labyrinthitis".
This is a condition which affects the inner ear, and causes nausea, dizziness and pain. Its extermely difficult to prove, too, that you havenot been suffering from it.
On the subjrct of self certification in the UK, you may self certificate yourself for up to 7 days. Any period of over 7 days must be certified by a doctor. These are the new department of health guidlines. Your GP can certificate you for periods of less than 7 days, should your employer insist, but its common now for GP's to charge for this service. It usually costs around £10 to £25.

Tell your boss you have a medical condition which is fairly urgent and needs treatment straightaway.
I'd say something like "I'm having my wisdom teeth removed" - because that'll have you out for 5 days, trust me I've been there.
Or you could say you're overly sunburnt.. depends what the weather's like where you live though.
Don't tempt fate by saying that a family member has died though - never advisable.

dont go for the family member is ill, cos that is tempting fate, dont do the old food poisoning one either cos after that long they might say ok you have to do a crap sample and that would just be like ewwwwwwwwwwwwwww.............5 days you would probably need a sick certificate though.....bad back, you can fake that at the docs, and no doubt you will have one when ya return from ya weekend away you lil minx you!!! Enjoy and do everything I wouldnt!! Wink wink xx

What about 'family problems'? You wont be required to give an explanation because no-one will want to ask what the problems are in case they upset you. When you go back to work you can just say that the problems are sorted and you would rather not talk about it.

Sickness and Diahorrea. Two days of actively puking/pooing followed by a day asleep, then two days recovering.
You've no idea what caused it but you still feel grotty.
No manager argues with the shits!

Be aware that if you have 5 days off, your Company can request a Doctors Certificate. If you cannot produce one (you can't if you're skiving!) they may be able to dismiss you (depending on your contract).

If your in the uk, then for 5 days sick you will need a sick note from the quack. You can self-certify for only 3 days.
