This seriously depends on the religion. In my faith we beielve the Relm predated this universe and Earth and lots of creatures live in it until the war broke out destroying their world forcing them into this one. My godmother is a nun and says before this Earth was made God had created another Earth for his angels, whom wared and destroyed it. Thus Lucifer was cast into hell, as he was the ring leader who was trying to over throw God. Either way, they still ended up stuck in this world LOL.

since a million years to us, is like a second to God, there is no telling. I believe He was just being God, and being worshipped by all his angels which He created because He is worthy of it all.

Some things we will never know, it was never meant for us to know, but I believe when our time comes, and when we see Him in Heaven, we will be given an understanding.

There was nothing. Only His Love. Anyway, I believe the world always existed, as a plan, in His mind. Somehow like this: I want to make a home. But when it starts to exist? When I start to build it or when I finished it? Hard to say, isn't it? I believe it start to exist when I got that house in my mind as a plan.
